SHDHS SUMMER CAMP REGISTRATION
Liability Waiver Consent
I, the undersigned parent/guardian, permit for my child to participate in Saint Henry District High School (SHDHS) summer camp. I understand that activities will take place on school grounds, including SHDHS’s main campus, the St. Henry Athletic Complex (SHAC), and/or other SHDHS-approved facilities, and that my child will be supervised by designated school personnel.
Considering the benefits of this activity, I agree to release, indemnify, and hold harmless Saint Henry District High School, the Diocese of Covington, and their representatives from any liability for accidents or injuries that may occur in connection with my child’s participation.
Medical Waiver Information
I acknowledge that if my child has a medical condition requiring health services and/or medication while attending camp, I will communicate those needs to school personnel before participation. In the event of an accident or sudden illness, I understand that reasonable efforts will be made to contact me or my designated emergency contact. However, if I am unavailable, I authorize school personnel to secure emergency medical care for my child.
I further recognize that I remain fully responsible for any legal liability resulting from the personal actions of my child while participating in camp activities.
Media Release Consent
I grant Saint Henry District High School permission to use my child’s name, photograph, and/or videotaped image in school publications, video productions, and/or school-affiliated websites. I certify that I am of full legal capacity to execute this authorization and release.
